What it means
Request rejected by Serving GW or PDN GW: the gateways refused the session; the GTPv2 cause in the Create Session Response says why.
Where you see it
In LTE NAS PDN connectivity reject, bearer resource reject or EPS bearer deactivation.
What to check
Look at the GTPv2 Create Session Response cause (e.g. 78 missing APN, 73 no resources).
Check the GTPv2 Create Session Response and the HSS APN configuration for the same APN.
